Medical Benefits
The company's medical insurance is provided by Blue Cross Blue Shield of TN. The company contributes 75% toward the employee only portion of the premium and 50% toward any dependents. Eligible employees include full-time employees actively working at least 30 hours per week. Employees must be enrolled for medical coverage before it can be offered to their dependents. Eligible dependents include the employee's spouse and children.

Dental Benefits
The Company's dental insurance is provided by Principal. This is a voluntary benefit election that the company will contribute $10 per employee per month toward dental insurance. Eligible employees include full-time employees actively working at least 30 hours per week. Employees must be enrolled for dental coverage before it can be offered to their dependents. Eligible dependents include the employee's spouse and children.

Life Insurance Benefits
Basic Life Insurance - Company Sponsored
The Company's basic life insurance is provided by GenericLife. The company provides employees basic life insurance in the amount of $20,000 plus two times an employee's annual salary rounded to the next highest $1,000. Eligible employees include full-time employees actively working at least 30 hours per week.
Voluntary Life Insurance
The Company's voluntary life insurance is also provided by GenericLife. Eligible employees include full-time employees actively working at least 30 hours per week.
